Cosmetic and Elective Surgery


Payment for these services MUST be made at least ten days prior to surgery.

Payment plans may also be available from your local bank, credit union, or other financial institution. Additional sources may be available on the internet. Be sure to consider all terms and conditions before applying for credit.



Other Treatment or Surgery


Payment is expected at time of service except for emergency treatment.

If you have traditional medical insurance coverage, we will, at our option and in lieu of immediate payment, file one claim for each treatment date to your primary insurance carrier. Payment in full is expected within 60 days from your insurance carrier, the patient, or the insured.

We are not responsible for claims which your insurance carrier does not process in a timely manner. Your coverage is a contract between the insured and the insurance company. We are not a party to that contract and have no legal authority to demand payment from the insurance carrier or consideration of the claim. We will provide you with documentation for you to file claims with secondary, non-traditional, auto, cancer, and other insurance carriers or third parties.

We do not accept liens or promises to pay based on potential lawsuits or "blanket claims" that someone else will be made to pay the bill.

We assume and act on the assumption that you intend to pay your bill in full and on time. The patient, insured, and/or legal guardian are ultimately responsible for payment of fees for medical treatment.

We will never refuse to provide treatment for urgent or life threatening medical conditions based on a patients ability to pay.





Important Notices

  • We reserve the right to refuse to file claims for procedures which, in our opinion, are cosmetic or elective.
  • Patients experiencing financial hardship must speak with the office manager on the first visit so that satisfactory arrangements for payment can be made.
  • Your health is our immediate concern. If you are truly indigent and unable to pay for urgent related medical expenses, talk to us. Payment plans are available.
  • Delinquent accounts are reported to the credit bureau and aggressively pursued.
